Contribution toward borrowing a breathing monitor

How do I apply for the contribution?

   Add your child insured with VšZP in the VšZP mobile app or in ePobočka to your account.

   Open a contribution request in Peňaženka zdravia — either in the mobile app or in ePobočka.

   Submit the request for your child.

   Fill in the required fields, attach proof of payment for the breathing monitor and the loan agreement, then submit the request.

   IMPORTANT! You must submit the request within 60 days of the payment date.

   The company ID (IČO) on the request is always that of Nadácia Križovatka: 42132380 (even if you borrow the monitor at a VšZP contracted facility).

   After your request is processed, you will receive a notification in the VšZP mobile app. VšZP will pay the contribution to the account you entered when submitting the request.
